Mayflower community performance Registration

    Please fill out one registration form per performer!  
    Note to Performers: 

    Please make sure to arrive no later than 1:45 PM so that the recital can start promptly at 2:00 PM.  Family and friends are welcome to attend.  Performers should come dressed in Recital Attire: no ripped jeans, flip flops, t-shirts.  Collared shirts are preferred.  Memorization is not required, but is encouraged.
